2025 Registration
& Waiver Forms

Registration forms and waivers are provided below for your convenience. Please note that these forms are date- and event-specific. Ensure that you select the correct link for your event(s).

• Event Registration Form: This will be released closer to the Round 1 event.

• Annual Forms: Pre-complete these forms and bring them to the event. Our registration volunteers will review and witness the forms at the first event you attend.

• Individual Event Waivers: Pre-complete these forms. Our registration volunteers will add event details, review, and witness the forms at the event you attend.

 

For all minor waivers:

1. The minor must complete their portion of the form.

2. The parental waiver must be completed and signed by both parents (if applicable). In cases of sole custody, the custodial parent or legal guardian must complete and sign the waiver. If there are legal restrictions on the non-custodial parent’s ability to sign, documentation may be required.

 

Forms can be provided to staff at check-in and registration on the day of the event.
